• Disproportionate breast implant size to chest width size.
• Excessive breast pocket dissection compared to breast implant size.
• Inappropriate placement location; choices being above, partially under the pectoral muscles or fully under the pectoral muscles.
• Unsatisfactory revision breast surgery.
